Treatment Foster Care

TREATMENT/THERAPEUTIC FOSTER CARE (TFC)
Treatment / therapeutic foster care involves intensive services provided with highly trained foster parents within the child and family team addressing the needs of children struggling with psychiatric issues involving severe emotional and behavioral problems. Therapeutic foster parents are required to have significant education requirements and are certified in medication management and crisis intervention. PATH staff offer counseling/therapy and support to the children in care and provide treatment consultation and behavioral management support to PATH foster parents. In addition PATH social workers offer case management with the child’s psychiatric community of care.

The treatment / therapeutic foster care program provides treatment services to all youth with an Axis I diagnosis in a foster family setting. Utilizing the WRAP Around process within the child and family team, the foster parents are a key provider of a therapeutic environment for the child.

COMMUNITY BASED TREATMENT PROGRAM (CBT)
The CBT Program is designed to utilize treatment foster care to provide community based services for children at levels of intensity designed to meet levels at Psychiatric Residential Care Facilities. Admission into the CBT Program must be approved through a Certificate of Need Process (CON) initiated through completion of and Admission Review. Continued Stay Reviews are completed 30 days following admission and as frequently as required thereafter. The CBT Program is up to 12 months in length and includes intensive community based services, monthly treatment meetings, crisis planning, case aide services, enhanced respite care and intense levels of individualized supports created to maintain the youth in the community.

FAMILY AND COMMUNITY EVALUATION PROGRAM (FACE)
The FACE Program is designed with the dual purpose of assessing a child's suitability for family and community placement and/or assisting youth who have completed their treatment program, but have difficulty transitioning to community living. The assessment provides recommendations for placement which may include treatment foster care, community-based treatment, group home, or psychiatric treatment care. FACE Programming is 90 days maximum and includes community-based services, crisis planning, and monthly treatment team meetings to evaluate a youth's ability to maintain placement in the least restrictive environment. Extra services may be added to aid the youth in maintaining placement. Certificate of Need is not needed for FACE Placements.

SPECIALIZED FAMILY CARE PROGRAM (SFC)
The SFC Program is a small therapeutic foster care program for youth adjudicated delinquent & under the custody of the Division of Juvenile Services. It is designed to be an aftercare program for delinquent youth with mental health diagnoses needing to transition out of the Youth Correctional Center (YCC) or other more restrictive placements to a treatment family foster care setting. The program serves 8-10 youth at one time.

PATH STEP DOWN FOSTER CARE
PATH Step Down Foster Care is equivalent to PATH Regular Foster Care. It is typically provided to youth when they turn 18 years of age and remain in foster care or to youth whose needs no longer require the intensity of services required for treatment foster care programming as defined by the child and family treatment team.

PATH REGULAR FOSTER CARE
PATH Regular foster care is provided at the request of the legal custodian. Services meet all the standards of the Council on Accreditation and mirror the services provided in Step Down.

PATH/DJS SEX OFFENDERS PROGRAM
This program is a treatment foster care service for youth with sex offending behaviors provided in a collaborative effort between DJS and PATH ND, Inc. The program is specifically designed for youth that have been placed in the custody of DJS, have a history of sex offending behavior and have an Axis I diagnosis, The youth served in this program are closely supervised in the home and community as defined by the youth's WRAP Around treatment plan. The youth will be co-case managed by the DJS and PATH workers. Foster parents involved in this program must be assessed and recommended by the PATH Regional Director.
